Step 1. Downloading Adobe Acrobat    
In order to view/edit this form you will need Adobe Acrobat Reader. If you already have Acrobat, skip to Step 2. Otherwise, click on the "Get Acrobat Reader" graphic above and follow the directions provided.

Step 3. Faxing Back Forms
Fax your completed form to our corporate headquarters at 314-531-0066. A Customer Service Representative will contact you promptly.

Merchandise Return Policy

A. Stock Merchandise
Normally stocked merchandise purchased from us may be returned for credit or exchanged with our prior consent and in some cases a restocking fee may be charged. The return must be made within 45 days of the original invoice date, and must be returned in resalable condition with its original packaging. Unless the merchandise was shipped by us in error, the transportation charges covering the return are the customer's responsibility. In unusual circumstances, when it is not practical for the customer to ship the merchandise back to us using a common carrier, we will pick the items up on our truck on a charge customer basis. The charge for this would be 10% of the merchandise value or $25.00, whichever is greater.
B. Non-Stock Merchandise
Return of non-stock items, items cut, modified, fabricated or manufactured to customer specifications will not be allowed.
